Effective Date: June 23rd, 2026

This Media Release & Content Consent Agreement (“Agreement”) authorizes Orisha Teeth Whitening to capture and use approved media content for business, branding, promotional, educational, and marketing purposes.

1. Consent to Capture Media

By signing this Agreement, I voluntarily authorize Orisha Teeth Whitening and its representatives, employees, contractors, photographers, videographers, marketing partners, and authorized affiliates to capture media involving me during or related to:

• Cosmetic services

• Training programs

• Certification programs

• Mentorship programs

• Educational activities

• Community participation

• Business events

• Product demonstrations

• Other interactions with Orisha Teeth Whitening

2. Authorized Uses

I understand and agree that approved media content may be used for:

• Marketing and promotional purposes

• Social media content

• Educational and instructional content

• Training and certification program content

• Business and advertising materials

• Testimonials and success stories

• Community and engagement content

2.1 Training & Educational Content Usage

I understand that media captured during training programs, certification programs, mentorship activities, workshops, educational sessions, student communities, or related educational offerings may be used for promotional, marketing, educational, instructional, business, certification, and training purposes by Orisha Teeth Whitening.

This may include photographs, videos, recordings, screenshots, testimonials, educational demonstrations, and related content.

3. Voluntary Participation

I understand that participation in photography, videography, testimonials, or media-related activities is voluntary.

Declining media participation will not affect eligibility for cosmetic whitening services or customer experience.

4. Ownership of Media

I acknowledge that all photographs, videos, recordings, edited content, training content, educational recordings, certification-related content, mentorship content, marketing materials, and media assets created by or for Orisha Teeth Whitening become the property of Orisha Teeth Whitening.

I waive any ownership interest, approval rights, royalties, compensation claims, or future payment related to authorized media usage unless otherwise agreed in writing.

5. Editing & Content Modifications

I understand that media content may be:

• Edited;

• Cropped;

• Enhanced;

• Reformatted;

• Combined with other materials;

• Used with graphics, music, captions, testimonials, or branding elements.

I waive the right to inspect or approve final edited materials prior to publication.

6. Cosmetic Results Disclaimer

I understand that cosmetic whitening results vary by individual.

Media content, before-and-after photos, testimonials, or promotional materials do not guarantee identical cosmetic results for other individuals.

7. Revocation of Consent

I understand that revocation of consent does not require the removal, withdrawal, recall, or modification of content and materials created, published, distributed, or used prior to my request.

This may include, without limitation:

• Educational materials

• Training content

• Certification-related content

• Instructional materials

• Student success stories

• Testimonials

• Marketing and promotional materials

• Social media content

• Photographs

• Videos

• Audio recordings

• Website content

• Printed and digital publications

• Business resources and archives

I further understand that such materials may remain in use where removal would be impractical, unreasonable, cost-prohibitive, or would negatively affect the integrity, continuity, educational value, historical accuracy, or business operations associated with such materials.

8. Social Media & Third-Party Sharing

I understand that media published online or on social media platforms may be shared, reposted, copied, downloaded, commented on, or redistributed by third parties outside the control of Orisha Teeth Whitening.

Orisha Teeth Whitening is not responsible for third-party use or redistribution of publicly shared content.

9. Minor Authorization

If the participant is under 18 years of age, a parent or legal guardian is required to provide written authorization below.

Parent/guardian authorization confirms permission for media capture and usage involving the minor participant.

10. Release of Liability

To the fullest extent permitted by law, I release Orisha Teeth Whitening from claims related to:

• Approved media usage;

• Promotional publication;

• Advertising usage;

• Online publication;

• Social media distribution;

• Editing or formatting decisions;

• Third-party sharing of publicly available content.
